Fire risk assessment
PHA is carried out in order to:
☑ Identify all potential hazardous scenarios;
☑ Eliminate Hazards wherever possible;
☑ Ensure that adequate protection is provided;
☑ Potential consequences are minimized;
☑ Correct equipment is installed as intended;
☑ Equipment meets legal requirements;
☑ The operation meets design intent;
☑ The means to bring process to a level of acceptable risk is identified
☑ An opportunity is provided to challenge the project definition, assumptions, and business value, with a competent team
A PHA Study may be initiated as a result of:
✓ A change as defined in the Management of Change (MOC) procedure,
✓ A new design of a plant/unit or facility
✓ A new or revised regulatory driver
✓ A PHA Revalidation Study,
✓ Following an accident which was unforeseen in prior PHA’s
